Welcome to Kaiser Permanente Southern California's Eye Monitoring Program

As a member of our program, you have full access to our services, which include:

* Active monitoring of your disease

* Periodic visits to your Eye Care Department for testing

* Access to answers about your eye condition

The purpose of the program is to detect changes in your condition that require medical evaluation and management. We hope you will appreciate the convenience and careful monitoring this program provides.
